Release and Waiver of Liability
I, the undersigned, being the parent or legal guardian of the above-named child, hereby give permission for my child to participate in the Kids Baking Classes organized by Cottian NJ LLC dba Patisserie Florentine. I understand that participation in these classes involves inherent risks, including but not limited to exposure to food allergens, use of kitchen equipment, and potential injuries.
In consideration of my child's participation, I hereby release, waive, and discharge Cottian NJ LLC, its officers, employees, instructors, and volunteers from any and all claims, liabilities, or damages arising out of or related to my child's participation in the baking classes. I understand that this waiver includes any claims based on negligence, action, or inaction of the released parties.
I acknowledge that I have read and fully understand this waiver and release of liability and that I am voluntarily signing it. I further understand that I am giving up substantial rights, including the right to sue.
Photo/Video Release
During this class, photos will be taken either by parents, kids, or Cottian NJ LLC. I accept and grant Cottian NJ LLC the right to take photographs and/or video recordings of my child during the baking classes. I understand that these images may be used for promotional purposes, including but not limited to social media, website, and printed materials.
Consent for Medical Treatment
In the event of an emergency, I authorize Cottian NJ LLC to seek medical treatment for my child. I understand that all reasonable efforts will be made to contact me before any medical treatment is administered.
Terms
- Payment: Due at the time of registration.
- Registration for the class is only confirmed upon receipt of full payment and e-mail confirmation.
- Spots in the class are limited and will be allocated on a first-come, first-served basis upon receipt of payment.
- All payments made for the class are non-refundable. No partial refunds.
- By making a payment, you acknowledge and agree that you will not be entitled to a refund under any circumstances, including but not limited to illness, schedule conflicts, or other personal reasons.
- Cottian NJ LLC reserves the right to cancel or reschedule the class due to insufficient enrollment, instructor illness, or other unforeseen circumstances.
- In the event that Cottian NJ LLC cancels the class, participants will be given the option to transfer to a future class date or receive a full refund.
- In the event of rescheduling, participants will be notified as soon as possible, and options for rescheduling will be provided.
- Enrollment in the class is non-transferable.
- Any payments not received by the due date will result in forfeiture of your reserved spot in the class.
- By making a payment for the class, you confirm that you have read, understood, and agreed to these payment terms and the non-refundable clause.

I understand that Cottian NJ LLC dba Patisserie Florentine reserves the right to terminate registration at any time and refund a pro-rated amount due to disciplinary problems.
I confirm that I am over 18 years old and the legal guardian of the child who will attend this class.
